Clinical Services Handbook

The Clinical Services Team at Sylvanvale provides therapy services for people of all ages and abilities. 

This Handbook explains our model of support and outlines the important information we need from you to help us understand your individual needs. This Handbook also sets out our polices, practices, procedures and additional information in relation to these services. When you sign a Service Agreement with us you are also agreeing that the policies, practices and procedures set out in this Handbook form part of our Agreement.

This Handbook may change from time to time, and when it does, we will let you know.
